Daryl and Daniel enjoy visiting jacksplace

jacksplace welcomes referrals from Berkshire, Dorset, Hampshire, Wiltshire and the Isle of Wight .  To meet our criteria young people must be under the age of 25 and diagnosed with a life limiting or life threatening condition as defined by ACT (Association of Children's Palliative Care).

Referrals can come from any source (e.g. the family, the GP, hospitals, consutlants or community nursing teams), provided it is with the knowledge and consent of the child's parent or main carer.

There is a formal process that each application must follow, to ensure that we are able to provide  the support each person requires.  The process is straightforward and does not usually take more than a few weeks to complete.  In emergency situations or at the end of a young person's life we can usually fast track the referral so that we can help as quickly as possible.
