Palliative care for children and young adults at Naomi House and Jacksplace
Families and young adults using Naomi House and Jacksplace do not have to pay for our services. However, for young adults who are referred from outside our area we will seek statutory funding to support their referral.
If you wish to make a referral to Naomi House or Jacksplace, please call our Care Team on 01962 760 555.
The Trust Fast Track Referral
If you have a baby, child or young adult who is likely to die within a short timescale (approximately 2 - 4 weeks), we can process a fast track referral, enabling you to use the hospices for end of life care.
A fast track referral can be made by telephoning our care team on 01962 760555.
To meet our criteria their condition(s) must fall within the guidelines issued by ACT (the Association of Children's Palliative Care). These fall into four main groups and are described below:
Group 1
Life threatening conditions for which curative treatment may be feasible but can fail. Where access to palliative care services may be necessary when treatment fails (e.g cancer, irreversible organ failures of heart , liver, kidneys.)
Group 2
Conditions where premature death is inevitable, where there may be long periods of intensive treatment aimed at prolonging life and allowing participation in other activities. (e.g. cystic fibrosis)
Group 3
Progressive conditions without curative treatment options, where treatment is exclusively palliative and may commonly extend over many years. (e.g. Batten disease, mucopolysaccaridoses, muscular dystrophy.)
Group 4
Irreversible but non-progressive conditions causing severe disability leading to susceptibility to health complications and likelihood of premature death. (e.g. severe cerebral palsy, multiple disabilities such as brain or spinal cord insult.)
