Community Support
The Hospice Outreach Team will consider a request for home nursing care from a family or from the professionals involved in the child’s care.
The service will aim to provide end of life care in the child home working in partnership with the nursing and medical services involved.
Crisis and emergency care will be considered if the family are experiencing an event or illness, which prevent them for caring for the child themselves. This will be for an agreed number of hours and number of visits.
Due to the large geographical area that our children and families come from, for practical reasons we need to restrict the home visits to within 30 miles of Naomi House. We will however consider any request for end of life care in the home.
Naomi House will know the child and family and an initial assessment of care needs will have been completed. Any new care needs can be discussed at the time of the request community care.
A risk assessment will be required before we provide care in the home. We will be grateful for your assistance in providing the information regarding the equipment in the home, and any other factors that are relevant to enable us to provide care in the home environment.
We will work in partnership with the professionals involved in the child’s care and follow the emergency plan that has been agreed by the family.
